Health Requirements
All students must be up to-to-date with physical examination and immunization requirements each school year. All completed forms must be emailed to the appropriate email addresses below or they can be turned into your child's school health office before August 1. Students who have not submitted their required health documents by the first attendance day of school may be excluded from school until notification has been received by Huntley 158 that the required health documents have been provided. Physical Exam Requirements
- All students entering early childhood, kindergarten, 6th grade, and 9th grade are required to have a current physical examination completed via the approved Illinois Department of Public Health Form. The parent portion of the form must be completed and signed.
- Students must have a current physical exam on file at school before they can tryout or participate in any extracurricular athletics. The physical must have been performed within the previous 395 days (13 months) prior to the start of participation, including tryouts and/or conditioning. Note: A sports physical does not meet the physical requirement.
- A current physical is one that was completed within one year of August 14, 2024.
- A copy of the physical examination and record of all immunizations the student has received must be on file with the school health office before school begins. Students failing to meet the immunizations and/or physical exam requirements will be excluded from all District schools.
- New to Illinois students will have 30 days from the enrollment date to submit documentation of the required immunizations and physical exam. All students who are new to Illinois schools also need a complete vision exam.
Immunization Requirements
- All students must be up-to-date with the State of Illinois immunization requirements.
- All students entering 6th grade must show proof of receiving one dose of Meningococcal vaccine (meningitis).
- All students entering 12th grade must show proof of receiving one dose of Meningococcal vaccine after the age of 16.
- All students entering grades 6 through 12 must show proof of receiving one dose of the Tdap vaccine.

Dental Examination Requirement
All students entering kindergarten, 2nd, 6th, and 9th grades are required to have a dental examination. Dental examinations are due by May 15th of the year the student is enrolled in kindergarten, 2nd, 6th, or 9th grade. A current examination is one that was completed within 18 months of May 15th of the school year.
Vision Examination Requirement
All students enrolling in a kindergarten program or enrolling for the first time in a public school shall submit proof of an eye examination completed within the previous year before October 15th of the school year.
