Health Requirements

To ensure health and safety for all, students entering early childhood, kindergarten, 6th grade, and 9th grade are required to have a current physical examination and immunizations on file in the school health office before the first day of school and preferably by August 1Students who do not submit required health documents by the first day of school may be excluded from school until documentation has been received per Board Policy 7:100

Students new to Illinois will have 30 days from the enrollment date to submit documentation of the required immunizations and physical exam and must also complete a vision exam.

Religious objections to any physical examination or immunization must be submitted by a parent/guardian in writing and must contain the required information. 

Health Forms 

Health forms can be found under the Students & Families tab of our website or may be obtained by a medical provider. Forms may be sent electronically (see school health office emails below) or dropped off at the school main office. Forms may not be dropped off at the district office. Note: Physical exams and immunizations can be submitted to the health office at any time during the current school year in preparation for the next school year.

Immunization Requirements

  • All students must be up-to-date with the State of Illinois immunization requirements. 
  • All students entering 6th grade must show proof of receiving one dose of Meningococcal vaccine (meningitis).
  • All students entering 12th grade must show proof of receiving one dose of Meningococcal vaccine after the age of 16. 
  • All students entering grades 6 through 12 must show proof of receiving one dose of the Tdap vaccine.

Sports Physicals

Students who wish to try out or participate in any extracurricular athletics must have a current sports physical exam on file at the school. The sports physical must have been performed within the previous 395 days (13 months) prior to the start of participation, including tryouts and/or conditioning. Note: A sports physical does not meet the physical requirement. 

Physical Exam and Immunization Requirements by Grade Level

Preschool Complete Preschool Physical Examination
  No additional immunization requirement if up-to-date.
Kindergarten Complete Kindergarten Physical Examination and all required immunizations including:
  Two doses of MMR vaccine
  Four doses of Polio vaccine
 

Four or more doses of DPT/DPTaP vaccine with the last dose being
received on or after the 4th birthday.

  Two doses of Varicella vaccine (chicken pox)
  Complete Kindergarten Dental Examination- Must be turned in prior to May 15.
  Complete Kindergarten Vision Examination- Must be turned in prior to October 15.
1st Grade No additional physical/immunization requirements if up-to-date.
2nd Grade No additional physical/immunization requirements if up-to-date.
  Complete 2nd Grade Dental Examination- Must be turned in prior to May 15.
3rd Grade No additional physical/immunization requirements if up-to-date.
4th Grade No additional physical/immunization requirements if up-to-date.
5th Grade No additional physical/immunization requirements if up-to-date.
6th Grade Complete 6th Grade Physical Examination and all required immunizations including:
  One dose of Tdap Vaccine
  One dose of Meningococcal vaccine (meningitis) after the 11th birthday
Note: If your child turns 11 after August 1st, you must submit proof of an upcoming scheduled appointment.
  Two doses of Varicella vaccine (chicken pox)
  Two doses of MMR vaccine
  Three doses of Hepatitis B vaccine at the required interval
  Complete 6th Grade Dental Examination- Must be turned in prior to May 15.
7th Grade No additional physical/immunization requirements if up-to-date.
8th Grade No additional physical/immunization requirements if up-to-date.
9th Grade Complete 9th Grade Physical Examination and all required immunizations.
  Complete 9th Grade Dental Examination- must be turned in prior to May 15.
10th Grade No additional physical/immunization requirements if up-to-date.
11th Grade No additional physical/immunization requirements if up-to-date.
12th Grade Must show proof of receiving two doses of meningococcal vaccine (meningitis) with the second dose received on or after the 16th birthday. If the first dose was given after the age of 16, then only one dose is required.


Dental Examination Requirement

All students entering kindergarten, 2nd, 6th, and 9th grades are required to have a dental examination. Dental examinations are due by May 15th of the year the student is enrolled in kindergarten, 2nd, 6th, or 9th grade. A current examination is one that was completed within 18 months of May 15th of the school year.

Vision Examination Requirement

All students enrolling in a kindergarten program or enrolling for the first time in a public school shall submit proof of an eye examination completed within the previous year before October 15th of the school year.
